Your care portal is built in your own Microsoft 365 environment — Teams, SharePoint, and Planner. If you ever stop working with us, you keep everything: your portal, your documents, your medication list, your provider directory. Nothing disappears.

What families ask before starting.

Do I have to commit to a tier right away?

No. Every engagement starts with a conversation. We help you figure out what level of support makes sense for your situation — and you can always adjust as things evolve.

What happens if my situation changes?

We can move up or down between tiers as your needs change. If you start with rapid advocacy support and the situation becomes long-term, we carry everything forward into a full coordination portal.

What does "setup" include?

Setup covers building your Microsoft 365 environment, provisioning your Teams workspace, SharePoint site, and Planner — and an initial intake session where we document your situation, care team, and priorities from day one.

What happens to my portal if I stop?

You keep everything. Your portal is built in your own Microsoft 365 account — not ours. When your engagement ends, your documents, medication list, appointment history, and care team directory remain in your environment. Nothing is deleted.

Do you work outside Massachusetts and Rhode Island?

Our clinical team is licensed in Massachusetts and Rhode Island. In Phase 1, we work with families in those states. If you are managing care remotely for a loved one in MA or RI, we can still help.
