Every engagement starts with a conversation. We will help you figure out whether rapid advocacy support, long-term coordination, or something in between makes the most sense for your family right now.
Rapid advocacy support is scoped to your situation. Pricing is based on complexity and engagement length. Contact us to discuss what your situation calls for.
A focused conversation to understand your situation, organize your concerns, and identify what you need most right now. We leave with a plan.
Help preparing questions, organizing your chronology, and navigating a hospitalization or care transition with structure. From day one of admission.
Making sure you understand what is happening, what comes next, and what questions to ask before you leave the hospital. Discharge is where things go wrong — we prevent that.
For families moving into sustained coordination — or transitioning from an acute advocacy situation into long-term support. Choose the level that fits where you are right now.
Your care portal is built in your own Microsoft 365 environment — Teams, SharePoint, and Planner. If you ever stop working with us, you keep everything: your portal, your documents, your medication list, your provider directory. Nothing disappears.
No. Every engagement starts with a conversation. We help you figure out what level of support makes sense for your situation — and you can always adjust as things evolve.
We can move up or down between tiers as your needs change. If you start with rapid advocacy support and the situation becomes long-term, we carry everything forward into a full coordination portal.
Setup covers building your Microsoft 365 environment, provisioning your Teams workspace, SharePoint site, and Planner — and an initial intake session where we document your situation, care team, and priorities from day one.
You keep everything. Your portal is built in your own Microsoft 365 account — not ours. When your engagement ends, your documents, medication list, appointment history, and care team directory remain in your environment. Nothing is deleted.
Our clinical team is licensed in Massachusetts and Rhode Island. In Phase 1, we work with families in those states. If you are managing care remotely for a loved one in MA or RI, we can still help.
Tell us what you are managing. We will help you figure out the right level of support — no commitment required.
